Highlight unreachable phones in Unified Setup phone selector drop-down

Issue description

Chrome Build: 72.0.3626.59

* What steps will reproduce the problem?
(1) Access the Unified Setup flow either as part of the OOBE or via Settings > Connected devices > Setup
(2) Expand the phone selector drop-down on Unified Setup landing page to view entire list of eligible Better Together hosts

* What happens today?
The phone selector drop-down can occasionally contains duplicates of the same device (see attached screenshot), making it difficult for a user to choose the correct entry. If the user accidentally selects a duplicate/stale entry, phone verification will fail to complete, and the feature will not work. 

* What is the desired result?
In the case of duplicate entries, we are technically able to determine which of the devices listed are online/available; practically-speaking, this can help users better decipher available vs. duplicate/stale entries, and choose the right host in the phone selector drop-down.

Per discussions with Chrome OS UX, please append " (offline)" to the device name of each currently-unreachable device, and grey the background of each unreachable line item using Google grey 600 (#80868B).
