Toggling "Disable Cache" leads to disk-cached file loaded without response headers

Issue descriptionUserAgent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10_13_6) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/71.0.3578.98 Safari/537.36 Steps to reproduce the problem: 1. Open Dev tools and go to networks tab. Select Disable Cache 2. Go to https://codeninja.co/ 3. Filter version.js in xhr 4. Uncheck Disable Cache 5. Reload page 6. Check version.js. File is loaded from cache but Response Headers are missing leading to it being blocked due to missing CORS headers. What is the expected behavior? Response headers should also be present when loaded from cache What went wrong? Response headers are missing when file is loaded from cache Did this work before? N/A Chrome version: 71.0.3578.98 Channel: stable OS Version: OS X 10.13.6 Flash Version: This issue only happens when playing with Disable Cache. If Cache is enabled from the start and cache is cleared, the file is loaded from disk cache with the correct headers. File where issue is observed is https://cdn.swaychat.com/v2/widget/assets/version.js

Also "Provisional headers are shown" is very annoying for debugging, please fix it.
