Fix crash when ble disconnect happens and device is NULL

In commit a7789a1c519ca4c84947690e6ff4c17c7d02c116, CHROMIUM: Call StartNotify with cccd_value and device_path, it add stop notify with device_path, but
in function clear_ccc_state, it may set device as NULL, which would
cause crash. This crash would happen when there exist le subscription,
and disconnect happens before unsubscribe.
