Eventually the fetch/resource_request.h top_frame_origin should be required instead of optional, and we should DCHECK that it exists when making a network service request.

This means making sure ever renderer method of creating a request properly sets it. A place to start is to look at all the ways that the request's site_for_cookies is set.
