What went wrong?
I had multiple drafts with documents attached. The docs had the same name, but were updated versions of the files. The attachments were saved in different drafts with the same file name (with different content) were overwritten and now the different files have the same content. I lost my updated work.

i.e. Document from 9.24 now has the same content as versions from 9.18 and 9.10. Documents have same name, but saved to different places in drafts.
