[Desktop PWAs] What should the window icon/title be set to (in- and out-of scope)?

Issue description

On Windows, Linux and macOS, we set the window title and (on Windows 7 / Linux) the icon. Currently the icon is the app icon, while the title is the page title. Seems inconsistent, especially when we navigate out-of-scope, and we end up showing a title for a completely different site.

I'd like UX input, but my suggestion would be:

- When in-scope, the window icon and title = the page's favicon and page title. (If it's too hard to use the favicon, we can continue using the app icon here.)
- When out-of-scope, the window icon and title = the app icon and app title. The page title will be shown underneath the window title in the CCT UI (Issue 853593).

Assigning to austinknight for UX advice on whether the above is satisfactory.
 
At first glance, this makes sense to me. Let's talk through it in our sync today, just to make sure I'm understanding correctly. But your proposal seems sound.
Decision: Always use app icon. But use page title when in-scope, app title when out-of-scope.
