Not sure why we are not showing disambiguation info there. What do you have in chrome://settings/autofill?

My guess is there's no disambiguation info because there aren't any other fields on the page. I think we only use fields for disambiguation if that info will actually be used to fill data on select.

Ideally we'd be deduping using only the fields that are actually present on the page, though.
