Screen reader indexes are missing/inconsistent in Customize and Control Chrome menu |

Issue descriptionGoogle Chrome 70.0.3524.2 (Official Build) dev (64-bit) Firmware Version Google_Lulu.6301.136.57 Steps to reproduce: # Turn on ChromeVox with ctrl + alt + z # Go to Customize and Control Chrome menu with alt + e or alt + f # Navigate down the list, paying attention to the indexes. Actual: New tab though Bookmarks read as 1 of 15 through 6 of 15. The word Zoom is skipped entirely and not read Zoom out read as 1 of 1 Zoom in has no index at all Fullscreen read as 1 of 1 There is no number 7 Print through More Tools read as 8 of 15 through 12 of 15 The word Edit is skipped entirely and not read Cut, Copy, and Paste are 1 of 3, 2 of 3, and 3 of 3 There is no number 13 Settings and Help are 14 of 15 and 15 of 15 Expected: consistent indexes - Number 7 and 13 should not be missing entirely - It is confusing that you would interrupt the middle of an index count with another set of indexes, only to resume again after that second set is done. - If cut, copy, and paste are 1, 2, and 3 of 3, then zoom out, zoom in, and full screen could also be 1, 2, and 3 of three since they are similarly grouped together. Recommendations from devs should be made about correct behavior since I can see several different ways it could be implemented correctly.

The menu items need explicit indicies. The buttons "make text smaller" ... are not menu items at all, so don't have indicies read. We're specifically looking for the posInSet and setSize attributes in AXNodeData in the mneu items here.
