[CRD iOS] Improve host setup instructions

Currently we have quite a few users complaining about confusions during the host setup process. The most frequent confusions are that they tried to enter IT2ME access code but couldn't find the place, and the screen doesn't go to the next page after the setup process is done on the host side.

We should improve our host setup instructions to make it more straightforward to the user.

It's also the time to update the URL in the setup instructions.

[CRD iOS] Improve host setup instructions

We have received quite a few feedback reports complaining about the
confusion in the host setup process. Some users set up an IT2ME host
instead of ME2ME host while others don't realize they need to refresh
the page after the host has been set up.

This CL improves the setup instruction by pointing the user to the
new ME2ME setup page, replacing confusing phrase, and telling people
to refresh the page.
