UI issues with site muting

UserAgent: Mozilla/5.0 (Windows NT 10.0; Win64; x64) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/68.0.3440.68 Safari/537.36

Steps to reproduce the problem:
a few UI issues with site muting.
Issue #1: an icon appears in the tab, but also the URL bar. This seems redundant and unnecessary.

 Issue #2 : those 2 icons don't even match! The one in the tab has a black cross through it, the one in the URL bar has a red X block badge on it.

What is the expected behavior?

What went wrong?
 Issue #3 : If you mute a site, then unmute it, the muted icon remains in the URL bar (see the 2nd screenshot)

Did this work before? N/A 

Chrome version: 68.0.3440.68  Channel: beta

Able to reproduce the issue on reported chrome version 68.0.3440.68 using Windows 10. As the issue is not seen in earlier versions(60.0.3112.0) hence considering it as Regression and  marking it as Untriaged. Adding Needs-Bisect label, will update the bisect info soon.

Able to reproduce issue on reported chrome version 68.0.3440.68 & on latest chrome 70.0.3501.0 using Windows 10, Mac 10.13.5 and Ubuntu 14.04 . Hence providing bisect information below.

another issue, similar to 785406, in attached screenshot, notice the video is paused and the audio indicator in the tab has disappeared (green arrow), but the URL icon is still there (red arrow)
