Clean up VRDeviceManager add/remove/initialized |

Issue descriptionRather than passing in three callbacks, use an interface. This interface may not directly have Add/Remove/etc. devices, but we will interact with providers differently to get their one device on demand. Related when cleaning this up, Add/RemoveDevice should use VRDeviceId, instead of unsigned int.
