Issue descriptionChrome Version: 69.0.3451.0 OS: MacOS 10.13.4 What steps will reproduce the problem? (1) Downloaded two extensions. (2) Only one icon appeared in wrench menu. (3) Tried to re-download disappeared extension. What is the expected result? My original two extensions appear in the wrench menu. What happens instead? Crash with the following information: [76977:775:0605/161808.967722:FATAL:toolbar_actions_bar.cc(226)] Check failed: model_->toolbar_items().size() == toolbar_actions_.size() (2 vs. 1) Please use labels and text to provide additional information. If this is a regression (i.e., worked before), please consider using the bisect tool (https://www.chromium.org/developers/bisect-builds-py) to help us identify the root cause and more rapidly triage the issue. For graphics-related bugs, please copy/paste the contents of the about:gpu page at the end of this report.

I can't repro this. tnijssen@ does this repro 100%? Is this with refresh or regular UI?

It only happened to me once and I can't repro it either. An extension I had installed just disappeared (and I don't know how or why). The check failed when I attempted to install a new extension afterwards. It was with the refresh UI.
