attestation: add support for the test CA

Also change the default certificate type to be a user certificate
to be consistent with cryptohome.

cryptohome: add per-PCA enrollment status

This changes allow us to include information about the new entities
introduced by CL:746146, namely identities and identity
certificates, into the TPM status message.

attestation: support CA type in creating cert requests

While there is no server implementation this brings the attestion_client
in line with cryptohome and lays the foundation for the CA type to be
used when adding support for multiple identities in attestationd.
