a11y: Chrome switch person dialog does not have meaningful title for screen readers

Issue description

Google Chrome 66.0.3349.0 (Official Build) canary (64-bit) (cohort: 64-Bit) 
Google Chrome 64.0.3282.167 (Official Build) (64-bit) (cohort: Stable)
Windows 10 Enterprise Version 1607		
NVDA 2017.4	
macOS Sierra 10.12.6 with VoiceOver 7.0	
Note: there has been a change in UI between 64 and 66 but this issue affects both builds.

To access the sign in dialog with the keyboard, use ctrl + shift + m

Windows and Mac: The switch person dialog does not have an accessible title. Only the buttons are read. The close button is not focusable
Mac:the frame surrounding the dialog lacks a meaningful title ""Frame 0"""
 
Related problem (please feel free to split into its own bug if needed)
While signed into Chrome, click on the identity button in the upper right corner	
Mac and windows: Bug: focusable controls do not have roles. They should be button or link
