a11y: ChromeVox is either misnumbering menu items or skipping menu items

Issue description

Filed on behalf of a user:
so, I notice that ChromeVox does not appear to be reading me everything in a menu. When I hit the more options menu in chrome I get a menu with 14 items, but chromevox only reads me the first 13. I've noticed this seems to be a problem with items that have submenus such as the help menu in chrome or if you hit chromevox m on the taskbar you get item one of three item three of three but the item two of three seems to be missing. again this has a submenu and i'm wondering if that's what's causing it.

Steps to repro:
# With ChromeVox running, open a new tab page
# Press alt+f to open the Chrome menu
# Arrow down through the menu, pay attention to the index reproted by ChromeVox. I.E. '1 of 14', '2 of 14' etc
# Notice that the screen increase, decrease, and fullscreen options do not have indexes, but they take up a single menu index
I.E. '6 of 14' increase, decrease, fullscreen, '8 of 14'
# Notice that certain areas of the menu start at a different index: cut, copy, paste for example
# Open the 'More Tools' submenu
# Arrow down through this menu and notice the item indexes reported by ChromeVox
Notice that index 2 is missing

Expected: Index announcements should be in sequential order, or if these sections of the menu are visually separated they should be announced by ChromeVox, or index values should not be announced at all.

Actual: Index numbers are missing or restart seemingly at random

yes, I can give exact instructions. I had sticky mode enabled and opened the file manager by hitting the searcher in the shelf.I then disabed sticky mode and opened the file manager. I then proceeded to re-enable sticky mode hit the tab key until i got to the list of folders on my device. for me it was 5. I then used the down arrow to navigate to add new services level 2. I pressed space to activate this option and I see a popup that expands visually but I can't get chromevox to interact with it. I notice this same behavior with other combo boxes such as on the web. I can see the box and it expands but I can't interact with the options inside of the box in order to make my selection.

This seems like a separate unrelated issue to the original reproduction. Combo boxes on the web are meant to be interacted with using the arrow keys (not with ChromeVox commands). This is because the page itself is in charge of what the arrow keys do. We can provide a ChromeVox hint to describe this interaction.
