MacViews a11y: focus lands on unlabelled image when installing an app

Issue description

When installing an app, the initial focus in the dialog lands on the unlabelled app icon image in the "Install to Applications" dialog.
 
Note: this bug was found by dsexton@ and leberly@ while testing the UI currently in M66 and above. 
Using macOS Sierra 10.12.6 with VoiceOver 7.0.
Google Chrome 66.0.3349.0 (Official Build) canary (64-bit) (cohort: 64-Bit) compared to 
Google Chrome 64.0.3282.167 (Official Build) (64-bit) (cohort: Stable)

Since this is a regression in accessibility, can we target this for M66?

I just tested this locally:

The existing Cocoa dialog has this bug (focus lands on the unlabelled app icon) but the Views version actually *doesn't* have it, so this bug is fixed by trunk MacViews. Marking this as Fixed.
Do we need to merge this to M66?  OR is the fix already present?
