When using auto fill, the "last name" field never autofill along with the rest of the profile

Issue description

UserAgent: Mozilla/5.0 (X11; CrOS x86_64 10032.86.0) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/63.0.3239.140 Safari/537.36
Platform: 10032.86.0 (Official Build) stable-channel auron_yuna

Steps to reproduce the problem:
1. Go to a form with the last name field
2. Autofill

What is the expected behavior?
All the fields are filled in

What went wrong?
The "last name" field is skipped, requiring the user to either enter the field manually or click on the field again to get the browser to fill in the field.

Does it occur on multiple sites: Yes

Is it a problem with a plugin? N/A

Does this work in other browsers? N/A

OK for some strange reason my name was split into two.  My first name as saved on a separate entry to my last name.  Deleting the duplicate and adding my last name to the entry fixed the problem.
