[desktop-pwas] Add Send Feedback button to DPWA uninstall

We should add a 'Send Feedback' button to the dialog for uninstalling Desktop PWAs so that we can get a perspective of what annoys users about the DPWA experience.

I was thinking it could be a:

"(Optional survey): Why are you uninstalling this app?

🔘 I don't need it any more.
🔘 It didn't work the way I expected.
🔘 I prefer to use it in a browser tab.
🔘 I don't want it opening the app when I click links on the web."

With a text field to send more feedback (or a button to open the Chrome OS Send Feedback pre-filled with app stuff).

We'd have to decide if we want to associate this with the app URL or just keep it general.

Another one we should have: "🔘 I prefer the mobile app." (or the mobile app is more functional, etc)
