AudioOutputStream isn't removed from the AudioOutputDispatcherImpl cache on error

If an AudioOutputStream calls OnError, we probably don't want to reuse it.

Just filing this so I don't forget about it.

AudioOutputResampler does remove these streams, but AODI may not. AODI is only used with Fake streams though?

Ah, right. The AudioOutputDispatcher class hierarchy got me once again. :)
