remove ContentVideoView

ContentVideoView has been replaced by AndroidOverlay.  since it's been like this for ~two release cycles, it's safe to remove ContentVideoView and any associated stuff.  also, remove the media switch UseAndroidOverlay (which chooses between CVV and AO).
