The USB hub had a faulty/flaky power connector, causing it to offline all devices. Replaced the hub, and verified all devices are now online again:

Note For the current ( #14170 ) build it looks like 2/3 devices are provisioning.

The last device too longer to recover/boot, so it won't be part of this run. It should be picked up in the subsequent run, however.
