Bluetooth audio output device ignored in WebRTC calls on OSX

Issue description

UserAgent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10_13_1) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/62.0.3202.94 Safari/537.36

Steps to reproduce the problem:
1. Using Apple Airpods connected via bluetooth
2. From system sound prefs: Output device: Airpods; Input device: Internal Microphone
3. From chrome sound prefs: Microphone set to 'Default'
4. Join appr.tc call

What is the expected behavior?
Audio output through bluetooth headphones

What went wrong?
Audio output through internal (chassis) speakers. System volume controls, including mute, have no effect on output level.

Did this work before? Yes Unsure - We started tracking this issue some time in M62, maybe 2-3 weeks ago.

* Ran the same test with a pair of Bose QC35 Wireless Headphones (with Mic, however unused for test). 
  - Result A: Subscribing to self in second tab: Same result as above
  - Result B: Subscribing to another machine, as in a normal 1:1 call: No audio output at all, rather than output through wrong device. Confirmed receipt of nonzero `audioOutputLevel` in chrome://webrtc-internals

So your setup is
*System default audio input: internal mic.
*System default audio output: Bluetooth.

When you run appr.tc the internal mic is correctly selected as input device, but all output is being incorrectly sent to internal speakers instead of the bluetooth device. Is this correct?

That's correct, although we are seeing varying results with different device configurations. In some cases (as in original report), output went to the wrong device, and we're also hearing reports of no audio output at all.

Just a point of clarification, this comment was buried in the original report underneath the gpu tab dump: 
I have seen this same problem with other Bluetooth devices. You should not need to find a pair of Airpods in order to see it for yourself. We can consistently reproduce with other bluetooth headphone + microphone combo devices, including:

* Apple Airpods
* Bose QC35
* Sennheiser Momentum M2 AEBT

Although the sample size is admittedly small, it's enough data to convince me this issue is not specific to a certain audio device model.

Can reproduce it on 62.0.3202.94 with Google-brand bluetooth headset.
I believe the different outcomes (audio output via built-in speakers vs no audio at all) depend on built-in speakers mute state (at least that's what I observed).

Can it be possibly related to  Issue 791899 ?
I checked and the problem is that output is being sent to the device associated with the input device.
It's not specific to Bluetooth since I can reproduce it with USB headsets as well.
 Bug 791899  is likely the same.
I bisected and this is a regression introduced by r493788. I'll look into it and will prepare a patch.
