FR: If the current tab URL is already part of an installed app, replace "Add to Desktop" with "Open in App"

Just an idea, since I often find myself in a browser tab of an installed app, to have an easy way to flip into the app view.

This would be the inverse of "Open in Chrome" that we have on the app menu. And since it's already installed, we shouldn't need Add to Desktop.

Just FYI, Add to Desktop might still be necessary for a while until we sort out our auto-update stuff, since it's the only way to generate a bookmark app from a newly scraped manifest. Maybe it's okay to say that if a user wants to update their PWA, they need to uninstall and reinstall?

We might also want to consider putting this underneath the PWA page action (if that's still happening).
