And, it happens on Chrome on both my current phone AND my previous phone. Same exact behavior, same exact fields. If I use Inspector to add the attribute autocomplete="off" to the text field the problem disappears.

It's all URLs with a form with name, address, etc. But here is an example
of a page with 2 forms:

When I type a letter into the last name box into "Form 1" everything
freezes for a few seconds. Sometimes it lets me keep typing after it
un-hangs, sometimes not. It's much worse on some letters than others. For
example, "S" almost always fails completely. The only way I can fill the
field is to type the word into another field and paste the whole word in at
one time. If I do happen to have a letter in the field, then focus it, the
browser hangs again for a few seconds.

Form 2 does not have any problems. It's almost like Chrome is getting hung
up trying to get the extended address information associated with the
autopopulation. The same thing happens on my desktop and on my phone.

Unable to reproduce the issue on chrome reported version 61.0.3163.100 and latest chrome 65.0.3316.0 using windows-10 with steps mentioned below:
1) Launched chrome reported version and add the addresses in chrome://settings/autofill
2) Navigated to URL provided in comment# 4 and try to add details in Form 1 and Form 2
3) Details didn't get populated for the provided URL

We have tried testing this issue with URL: https://bootsnipp.com/snippets/featured/register-page, with this URL, the details get auto populated as details given in auto-fill, the browser didn't get freezes.

Please find the attached screen cast for your reference and let us know if we missed anything in reproducing this issue. Could you please try to test this issue by creating new person with no extensions and apps in it.

As I mentioned, the field MUST be a last name field. I suspect that those of you who have tried to reproduce this are trying with a clean, or relatively clean install. This problem only developed after using the installation for a long time. 

What can I do to demonstrate the issue for somebody? Would anybody be willing to share a screencast? (yes, this issue so annoying for me that I'll willing to go to that extent to get it confirmed...)

I added a rotating box using CSS that will freeze when Chrome freezes. Even though you cannot see the pulldowns in the screencast, each time I type into the text boxes the autofill options are populated. 

In Form 1, certain letters take longer than others to populate. For example, A takes a while, whereas Q is quick. S freezes the browser for so long it makes things unusable.

Form 2, which does not have address fields, does not exhibit this behavior.

This same thing happens on my desktop AND my phone on EVERY last name field. It happened on my old computer as well, then on my current computer after a year or so.

Unable to reproduce the issue on Win-10 using latest stable #64.0.3282.140 and latest canary #66.0.3336.5.

Following are the steps followed to reproduce the issue.

1. Navigated to http://concepts.waetech.com/formfreeze.cfm
2. Typed letters in the last name field and observed no freeze.

When I switch to "Guest" on my browser the problem does not occur. 

I am using: Version 64.0.3282.140

The tests that are being executed are likely not simulating having a VERY VERY large autofill database. On my system I have filled out thousands of forms with names and addresses. This stall only occurs when I type into a field that would traditionally be a last name field (e.g., named "last_name") AND when the form also contains address fields. The letters with higher usage, such as S, stall more than letters with limited usage, such as Q.

There is clearly a blocking issue relating to the lookup of the names for autofill. I would be happy to share my desktop in real-time with someone who might be able to address the issue.

As a test I also turned off autofill in the Chrome settings. As expected, the delay did not occur.

As the issue is not reproducible from TE end(as per comment#6,11), Could someone from UI>Browser>Autofill team please help in triaging this further.

As per comment#15, issue is not reproducible from TE end, hence adding 'TE-NeedsTriageHelp' label to remove this bug from TE bucket and requesting someone from UI>Browser>Autofill team have a look at this issue.
