MD Extensions: extensions on/off state slightly unclear in grid view

Issue description

Because there's no textual label for the toggle button in extensions, some things are confusing:
1) in general, the user might not know what that toggle is for, or what it indicates.
2) the "on but disabled" state is extremely confusing, since the toggle is gray, it can easily be confused with "this extension is off"

Proposal: Append a "(disabled)" suffix to the name of a disabled app/extension to make it more clear, see screenshot.

Also, another problem I am seeing is the fact that the entire card gets opacity of 0.6 when disabled, which includes the toggle control that the user can use to re-enable the extensions.

The bottom row should not change opacity IMO (including Details button and toggle), since the user can still use those when an extension is disabled.
