Observed this on a developers machine. I cannot repro locally on stable or canary.

It was also observed when we saw TWO logs of "service worker activated" on each reload.
In this case, I did not see multiple active worker versions. I did see many redundant/stopped workers in service-worker-internals, but that was it.
