Cannot Install recovery media onto a chromebook that was running version 59

Issue description

UserAgent: Mozilla/5.0 (Windows NT 6.3; Win64; x64) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/60.0.3112.101 Safari/537.36

Steps to reproduce the problem:
1.Create recovery media via usb
2.Plug USB into device
3.Verifies media
4. installs 

What is the expected behavior?
Installs the new media and reboots

What went wrong?
It stays on installing media screen for an indefinite amount of time.

Chrome version: 59.0  Channel: stable
OS Version: 
Flash Version: 

I've spoken with some other schools with these devices, and we have noticed a significant number with these two particular issues

Confirmed with >5% of Glimmer units in my organization.
A Glimmer Chromebook will be presented to Support with an OS Error or black screen. Recovery mode seems to function properly, but never completes. "System recovery is in progress..." is displayed "forever" (tested overnight).
See attached for a screenshot of VT3 output from an affected Glimmer (Lenovo Chromebook 11e) recovery run.

Seeing a high volume of this with the 11e both the 20DU0008US and touch 20DU0003US version.  Re-imaging the system using the recovery utility generally does not help, but running the manufacturer RMA shim corrects the problem more often than not (are a lenovo service partner)  In the scenario that the shim will not correct the error will change from no bootable kernel, to no bootable disk found after the shim fails.  Is the MMC getting corrupted by an update?  We are also seeing a high volume of failures with the N series chromebooks that cannot be recovered by the recovery utility or RMA shim.

has anyone looked into this?  Still seeing a steady stream coming in for this issue each week.
