Plugin blocked icon showing in Omnibox even though Flash content is running correctly

Issue description

UserAgent: Mozilla/5.0 (Windows NT 6.1; Win64; x64) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/60.0.3112.101 Safari/537.36

Steps to reproduce the problem:
1. Enable Flash and set to always run 
2. Go to a site that uses Flash, http://get.adobe.com/flashplayer/about/
3. Flash content will load and run normally but the error still appears in the address bar. 

What is the expected behavior?
No errors in the address bar 

What went wrong?
The Blocked puzzle piece icon still shows in the address bar even when Flash content is running normally. 

Did this work before? N/A 

Chrome version: 60.0.3112.101  Channel: stable
OS Version: 6.1 (Windows 7, Windows Server 2008 R2)
Flash Version: Shockwave Flash 26.0 r0

On this particular site, even if the main flash content is running, there is a smaller piece of Flash content that is blocked. It's a good thing that it doesn't bother the user in this case -- but we show that icon when ANY Flash is blocked.

Is there a place you can check to see which Flash content is blocked?  Is this related to Flash content under 300x400 pixels?  I ask because our company's website sees this, and we aren't aware of any flash content under that size being displayed.
