Add node counter per audio context

Issue description

When DEBUG_AUDIONODE_REFERENCES is set in WebAudio, a global counter is created that keeps track of how many handlers of that type are alive. It is incremented when a handler is created and decremented when the handler is destroyed.

This is global so it includes information about all contexts.  It would be very useful for tracking leaks if each context also had its own counter to keep track of the handlers associated with the context.
