Videos hanging/stalling/pending when played via JS API

Issue description

UserAgent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10_12_5) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/59.0.3071.115 Safari/537.36

Steps to reproduce the problem:
1. Navigate to 'edits' section of website, roll mouseover titles to play videos
2. Click a video to load an enlarged version
3. Check out the dev tools to see a load of pending requests.
4. Repeat test in Firefox to see intended behaviour (Chrome was the same when the site was built late 2016)

What is the expected behavior?
The videos should load and play smoothly without significant hanging.

What went wrong?
The videos hang for long periods of time, it appears the browser is making unneeded requests and then waiting for them.

I think this is related to broken sidx processing in ffmpeg as used by Chrome; sorry for the trouble, we're looking into it:  issue 738595  and https://trac.ffmpeg.org/ticket/6402

Firefox is still working in the way Chrome used to. I realise this website is rather unusual in the way it uses video so appreciate the work on this.

I've tested 63.0.3212.0 and issue is still present.

Hmm, that's not good, I thought something I checked in to 63.0.3208.0 would have fixed this. I will have another look. Does it behave any differently from before?

Ah, sorry, I'm confusing this issue with another, but will have a look either way.

The behaviour is unchanged. I've just tested in 64.0.3248.0 as well and it's the same : (
Just tested in 65.0.3287.0 and there is a marked improvement - still not quite as smooth a transition between SD and HD versions of the films on my test case but much better performance overall. Thanks!
Hmm I'm surprised M65 has changed anything. We haven't made any changes there AFAIK, the biggest update would be in the most recent M64 versions. M64 should show up on beta channel soon if you can try there again, the version should be higher 64.0.3278.0.

I think the only way to resolve this 100% is for Chrome to ignore some of the boxes or to mux your mp4 clips for progressive using -movflags faststart from ffmpeg.

If you need them to be valid fragmented MP4, you need a global sidx box without an mfra at the end, which sadly no off the shelf tool will do for you currently. I've been meaning to contribute support for this to ffmpeg, but haven't got to it yet.
This seems to perform pretty well in M65, with additional improvements in M66 from this CL:
