TreeScope should have live Range objects list for relocation instead of Document

Because of Range objects can not cross tree boundaries, changes of the
tree can not affect Range objects in other trees.

On Polymer we were brainstorming using Ranges to keep track of dynamic parts of templates after they had been stamped out, but the cost of keeping many live Ranges in a document would be prohibitive right now.
