OnCreated is currently called after SetWebContentsTitleForAudioLogEntry, so the log call from SetWebContentsTitleForAudioLogEntry may arrive first, at which point it is considered to arrive after the stream is closed. This will drop setting the title. The reason this doesn't always happen is that SetWebContentsTitleForAudioLogEntry will hop to the UI thread to get the title first.
