Make sure we are not parallelizing the download of cached images

We need to make sure that we aren't inadvertently parallelizing images that are being pulled from the cache.

We recently found the image download bug is caused by history db. 

The possible cause is some one has a migration step on history db to copy every URL records to a temporary table and copy back, which may cause the history db explode.

This commit adds metrics for history db migration failures.
