Remove Chrome OS banner and replace with the Add to Shelf dialog

Use the same modal dialog as when you choose "Menu -> More tools -> Add to shelf" for the site-triggered install flow (BeforeInstallPromptEvent.prompt()). Remove the more subtle info-bar.

This is blocked on  Issue 729919  because we would not want to show modal dialogs without the site explicitly opting in.
