native heap profiling symbolization: Convert unknown symbols to <dll> + offset

Original idea suggested by etienneb. This allows us to diff memory dumps across multiple runs of Chrome [e.g. undoes the effect of ASLR]. This is useful for comparing two traces from the perf waterfall from different runs of Chrome, without having system symbols.
