Pick some sdhci CRC fixes to 4.4 kernel

These three patches seem like they should be on our 4.4 kernel.  I'm a bit confused why they were tagged as 4.5+ upstream:

ec014cbacf62 mmc: sdhci: clean up command error handling
96776200898c mmc: sdhci: move initialisation of command error member

I don't think they are terribly important for our use cases, but I noticed them in the context of  bug #721944
