Regression: [Harmony] Extension icon is not seen on ‘Re-enable new permission’ bubble.

Chrome Version:60.0.3094.0 (Official Build) 53f4c2d32725416af10011d5db44f292d6e69fd2-refs/heads/master@{#470111}
OS: Win(7,8,10),Mac (10.12.3,10.11.6),Linux

Pre-condition:
1. Enable Material Design in the rest of the browser's native UI flag
2. Sign-in the browser with valid credential which contain sync extensions in disabled form for example ‘Black Menu for Google' extension.

What steps will reproduce the problem?
1. Launch chrome and click on wrench yellow colour icon.
2. Select extension is disable option from wrench such that prompt is seen.
3. Observe bubble.

Actual: Extension icon is not seen.
Expected:Extension icon should be seen.

this is regression issue, broken in ‘M 59’ and below is manual bisect info:
Good build:59.0.3044.0
Bad build:59.0.3046.0

Using the per-revision bisect providing the bisect results,
Good build: 59.0.3044.0 (Revision: 457648).
Bad build : 59.0.3045.0 (Revision: 457952).

You are probably looking for a change made after 457773 (known good), but no later than 457774 (first known bad).

This is currently still in flux. The mock for this dialog doesn't show an icon and the title and text are going to be different.
