Clean up QuicStreamFactory::active_jobs_ and friends

Code support to load server info from disk has been deprecated recently. 

A more deep cleanup in QuicStreamFactory is needed, active_jobs_ could be changed to a 1-1 map from ServerId to Job rather than a map of Jobs to improve the code readability as well as memory efficiency.

NetLog support can also be cleaned up.

Do you want to merge this into  Issue 707953 ? Ryan stapled his CL on that bug. It might be better to have everything at one place.
