Include more info for every MEDIA_LOG(xxx) event, and use it in MediaError.message

To make the error details in MediaError.message more understandable, let's include things like:

Module tag (e.g. "AVDA", or possibly "mp4_stream_parser.cc")
Function information (e.g. media::Mp4StreamParser::EnqueueSamples(...))

Of course, don't log pointers, media stream names/URIs, etc.
