Password bubble is also shown at similar spot just under where the app menu button would have been if it were not hidden. (Using https://codereview.chromium.org/2808823002)

Since chrome://flags/#secondary-ui-md is enabled in the original report, I'd say this is a MacViews issue.

Currently permission prompts anchor to the location bar or the app menu depending on which one is present, in this case neither are. Perhaps there should be a third default in the top center overlapping the title bar.

If you've enabled all the flags in #1, there should be an option in the three dot menu under "More tools" to add the site to Applications. Otherwise: 1. Enable chrome://flags/#enable-desktop-pwas 2. Go to https://permission.site/ 3. Open three dot menu > Create Shortcut... 4. Go to chrome://apps/ 5. Right click newly installed shortcut and click "Open in Window" 6. Open app

#12: Thanks, those steps work - the flags in the original post do not cause a new menu item to appear but I can repro this issue using the steps you gave. I'll put up a fix.
