Add form interactions UKM

We want to log from interaction events - specifically |AutofillFormSubmittedState|,
|UserHappinessMetric|                                                                                                            |FormEvent| for address                                                                                                          
|FormEvent| for credit_card                                                                                                      
|UnmaskPromptEvent|

so we can understand why auto-fill usage rate is low based on URL.
