These providers are created by MediaStreamManager and lifetime management would be simpler if they were exclusively owned by MSM.

It doesn't really make much sense for MediaStreamProviders to outlive the MediaStreamManager, and several uses of refcounting could be replaced with weak pointers.

Issue 526638 is an example crash whose analysis is made more difficult by having VideoCaptureManager as refcounted.
