Authentication required popup can be repeatedly raised by a malicious site without the possibility of dismissing it

Issue description

UserAgent: Mozilla/5.0 (Windows NT 10.0; WOW64) AppleWebKit/537.36 (KHTML, like Gecko) Chrome/56.0.2924.87 Safari/537.36

Steps to reproduce the problem:
1. Create a malicious website that requires authentication and repeats requests forever

What is the expected behavior?
"Prevent site from generating more dialogs" checkbox should appear

What went wrong?
There is no way to prevent the site from generating infinite authentication required dialogs

Although annoying, we treat these kinds of denials as functional issues, rather than security issues per-se.  Removing restrictions.
