I'm getting incomplete addresses through the form at the end of this Squarespace-based page <http://www.pbjameson.com/blog/2016/10/13/wash-your-hands> and I can see why when I try to fill out the form myself in Chrome Version 54.0.2840.71 (64-bit). I start typing my address, the autofill drop-down appears, I select the address I want, but it doesn't stick. Really, it should appear when I autofill my name or any other field on the form, but the address fields don't seem to be acknowledged by autofill at all. The field only retains the numbers I keyed in, and I imagine people are moving on after they think they completed the address with Autofill. It works properly in Safari. Squarespace rep says it's a problem with Chrome. I have tried playing with the fields on the form in Squarespace and Chrome Autofill just isn't picking up whatever contextual clues are necessary to fill out the Address field(s). When I had two separate street address fields created in two different ways possible on Squarespace, just for the sake of testing various configurations, Autofill did recognize one of them, if it was in a certain order. But if the fields appeared in another order, the Address 2 field would populate with the street address, while the City, State, etc. wouldn't populate at all. Maybe there's some clue in that behavior? There's some interplay between how Chrome's Autofill is working with the Squarespace form. Squarespace says the form is working as intended but it's unclear why Chrome isn't adapting despite all their efforts to make clear which field is which.
