Bad Refresh rate for secondary external monitor(4K) on Panther

Issue description

Chrome Version: 58.0.3029.12
Platform: 9334.5.0 (Official Build) dev-channel panther
Firmware: Google_Panther.4920.24.26

What steps will reproduce the problem?
(1)Connect both the monitors to the device (1080 (primary monitor) & 4k) and sign in 
(2)Stay in any mode (Extended or Mirror)
(3)Unplug & Plug the 4k secondary monitor

What is the expected result?
No Flickering should be seen   

What happens instead?
Bad refresh rate and the screen flickers (tried with different resolutions, still see the same).

4k Monitor - HP Z 32x-HSTND-9081-A
Was able to repro 3/5 times. 

For graphics-related bugs, please copy/paste the contents of the about:gpu
page at the end of this report.

 
I can repro it in all these scenarios:
1. unplug/plug
2. Reboot
3. Suspend/Resume the device
1. Secondary 4k monitor connected with HDMI cable
2. In comment #2 - Primary monitor connected with HDMI cable
