[ControlledBy] show extension name and disable button when setting controlled by extension

In chrome://settings (md-settings) in some places we show the extension name and a disable button when an extension is controlling the settings. In other places just an icon with a tooltip is shown.

Chrome will currently show one or the other to match the old options page, but can it be better? With effort, the extension and disable button could be shown in all cases.

The screen shots above show: 
- a setting (not shown) that is controlled by the BeyondCorp extension with the disable button.
- The javascript content setting is controlled by an extension, but the particular extension is unspecified.
