Table header is duplicated on print

UserAgent: Mozilla/5.0 (Macintosh; Intel Mac OS X 10.11; rv:54.0) Gecko/20100101 Firefox/54.0

Steps to reproduce the problem:
1. Open the test case
2. Open print preview
3. Set the followings:
  - Destination: Save as PDF
  - Layout: Portrait
  - Paper size: A4
  - Margin: Default
  - Headers and footers: Off
  - Background graphics: On

What is the expected behavior?
The table header ("HEADER") is displayed only once

What went wrong?
The table header is duplicated and displayed twice

Did this work before? N/A 

Does this work in other browsers? N/A

Chrome version: 56.0.2924.87 (Official Build) (64-bit)  Channel: stable
OS Version: OS X 10.11
Flash Version: Shockwave Flash 23.0 r0

When I saw it, my settings were slightly different than OP's. I had:

  - Destination: local printer
  - Paper size: don't remember, but not A4
  - Background graphics: Off
