Add overflow menu

Overflow menu UI provides a place to put relatively uncommon-but-useful functions such as refresh, forward, etc. Per bug 692336, this would be accessible via the menu UI. We may also in the future explore making it (or some of it's contents) accessible via a fast-path gesture.

Not that for FF release, we can use an empty placeholder UI for this mode.

UX has approved closing these bugs.  As tasks are scoped, these bugs can be reopened (if relevant), or recreated as needed.
