a11y: FOcus should return to source element

Issue description

Changing settings
FOcus should return to source element (e.g. open dialog > close > focus goes to main menu)

Chrome Version: Chrome 58.0.2994.0
OS: Windows 10
Screen Reader: NVDA 2016.4, JAWS 18.0.2324

What steps will reproduce the problem?
(1) Install JAWS via http://www.freedomscientific.com/ or NVDA via http://www.nvaccess.org/
(2) chrome://md-settings
(3) Activate *ANY* button > BACK button to return

What is the expected result?
Focus to return to source control that opened dialog

What is the observed result?
FOcus goes to top (main menu)

It makes sense that when a dialog closes, the same element that was focused should be focused again. I'll look into restoring focus when a cr-dialog closes. This behavior will need to be override-able b/c in some places the dialog is shown from an overflow menu. In that case we want to focus the overflow button and not the menu item that triggered the dialog.

For subpages, this is a little bit more gray. A sub page behaves like a dialog to someone someone who's using a screen reader, but it's very different. Does it make sense that we change focus here?
