Entering New Contacts

Issue description
When I add a new contact and save, sometimes the name ends up disappearing and the email address shows up in the "add name" field.

Version 56.0.2924.76 OS Sierra 10.12, Windows 10, Steps to produce: create a contact in that is in our company directory. When I type in the first and last name, email address, etc., once the contacts saves, sometimes the first/last name disappears and replaces it with the email address as the first/last name. this may happen when I am using a name that is listed/linked in our global directory. Not sure.

Tested with sample url(http://quickui.org/demos/contacts/) in chrome # 56.0.2924.76,Stable #56.0.2924.87 and Canary #58.0.3020.0 on Mac 10.12.3 & win 10.0 and not able to reproduce the issue.Please find the screen cast for your reference. @ jed.hammel: Could you please let me know if I have missed anything and if possible, can you please let us know the URL (domain) in which you are facing the issue and if it can be accessed globally outside your company. If possible, can we get an access (Test account) so that we can give a try from our end. Thanks in Advance.
